Please complete this Release and Waiver form prior to participating in any activities with Gnosis Wellness Collective (“Gnosis”). This document affects your legal rights—please read it carefully. By signing below, you acknowledge that you are voluntarily participating in activities that involve physical movement, dance, yoga, and related wellness practices, and you assume full responsibility for your participation.

Assumption of Risk

I, the undersigned, acknowledge that I (or the minor on whose behalf I am signing) have voluntarily chosen to participate in movement-based, dance, and/or yoga activities conducted by Gnosis Wellness Collective. I understand that these activities involve inherent risks, both known and unknown, that cannot be completely eliminated regardless of the care taken to avoid injury.
